How can I become a customer and begin ordering?

A standard credit application must be completed in order to become a customer. The information we require includes: Company name, DBA (if different), contact person, address, phone, fax, accounts payable contact person, federal tax ID, date business was established, type of business (corp., partnership, etc.), names of three chief officers, trade references with addresses, bank reference and signature.
